COVID UPDATE: 09/19/2020

To all Residents and Families 9/19/2020

As of 9/19/20 the facility has confirmed a resident who has tested positive for COVID-19. This individual is being cared for safely in regards to state and federal guidelines. We continue to monitor all residents and employees for any symptoms. We aim to provide the best quality of care for your loved ones and to ensure their overall safety.

All employees and residents in the facility will be tested for COVID-19. We will continue to keep you updated with any new confirmed cases.

We are allowing essential and compassionate caregivers to visit. These visits can be scheduled by contacting us at (904) 714-3793. Visits are 30 minutes long and must be scheduled in advance. End of life visits are allowed 60 minutes. All visitors must be symptom free, educated and trained on Infection Control procedures before entering the facility.

We will notify everyone when general visitations are allowed. Due to the Governor’s executive order the facility must be free of COVID-19 for 14 days before resuming general visitations.

COVID UPDATE: 09/14/2020

To Family Members & Loved Ones: 9/14/2020

First I want to make you aware of some federal changes that came from the Centers for Medicaid and Medicare also known as CMS. CMS is our federal regulatory agency. We have to comply with their regulations in order to provide services to our Medicaid and Medicare residents. To give you the short form, basically we are required to test our staff based off the county positivity rate. Currently Duval County is at 6.9%. With that positivity rate we are required to test our staff weekly. If we have any staff that come back positive we would then be required to test all staff and residents within 7 days and every 7 days until we go 14 days without a positive result. If the county positivity rate goes above 10% we would be required to test twice a week and if it goes below 5% for 14 days we have to test monthly but the same rules apply in regards to a positive result which requires us to test all staff and residents every 7 days until 14 days without a positive result. But keep in mind the State requires us to test every two weeks so we will always test every two weeks. Even if our county positivity rate goes below 5% we must follow the more stringent of the regulations between state and federal regulations.
Currently we have identified 3 new positive cases as of 9/13. We continue to test all employees and will test all residents. We have taken all necessary precautions to ensure everyone’s safety. Testing will take place on 9/14 & 9/15.
ALL licensed nursing homes have to follow both the Federal and State regulations
We understand how important it is to be able to visit with your loved ones. On September 1, Governor DeSantis lifted the visitation restrictions that had been in place to protect long term care residents from COVID-19. In order to keep the safety and well-being of all as a top priority, the state’s new policy establishes specific guidelines that must be followed. The Emergency Order can be found here.
We have implemented appropriate policies to comply with the Emergency Order so you can visit with your loved ones in the safest environment possible. This policies includes, among other items:
● 14 days without a COVID positive resident or staff for general visitations ONLY
● Precautionary infection control measures
● Visitor screening measures
● Policies and Procedures developed
● Education developed for visitors regarding PPE, hand hygiene and social distancing-in the Nursing Home Industry
● Resources allocated for visits
● Schedules for visits-agreeable to both parties-by appointment only
● Visitations being monitored
● PPE supplies assessed to make sure we can accommodate
● Designated areas assigned for visitation
● Consents for visitors to sign stating they understand the infection control practices and visitation policy
● Social distancing and mask will be required
● Disinfection and cleaning between each visit
● No more than 2 visitors at a time

As of 9/14 we are only allowing essential/compassionate caregivers to visit their loved ones. These visits can be scheduled through our Activities Director who can be reached at (904)-714-3793.   • Consents need to be developed for visitors to sign stating they understand the infection control practices and visitation policy
  • Social distancing and mask will be required
  • Disinfection and cleaning between each visit
  • No more than 2 visitors at a time

COVID INFO: 09/08/2020

Today’s announcement by the governor is a significant step forward in achieving our shared goal of reuniting our residents with their loved ones. We know you are looking forward to being reconnected with your loved one in person and like you, we look forward to returning some sense of normalcy to our center so our residents can once again experience it as the place they know as their home. We are in the process of putting the policies in place to reopen safely to visitors based on the state’s guidance and will communicate those procedures very soon.

In the meantime, please know that our focus will remain on keeping our residents and staff safe from the virus, which has been our number one priority since this pandemic began. If you have questions, please contact facility.
